Once the weapons or spells have been assigned equip them to the hands you wish to use them with, then go into the group in question & hit to save Weapon/Spell hand assignments (for dual-wielding both items need to be equipped when you do this as otherwise it will clear the hand not holding something from that group).
Hit again to assign that item to that group.
